Fractional laser treatment has bridged the gap between the ablative and non-ablative laser techniques used to treat sun-damaged and aging skin as well as acne scar dyschromia and other skin conditions with great success. Whilst ablative laser treatments work mainly on the epidermis (surface skin cells) and non-ablative treatments work solely on dermal collagen (mid-layer of skin) only, fractional laser treatment works at both the epidermal and dermal layers of the skin. Fractional laser treatment is unique, since it is designed to have capabilities approaching ablative laser resurfacing results but without the discomfort and inconvenience of a lengthy downtime and with a minimum of risk.

Fractional Laser treats a small "fraction" of the skin, (usually 15-25% coverage) leaving intact, undamaged skin around each treated area (microthermal zone) to act as a barrier and a reservoir for rapid healing. As the treated area heals, fibroblasts in the skin are stimulated to naturally remodel the skin structure (due to dynamic thermic effect of laser) to create firmer smoother healthy looking new skin thus enhancing overall appearance. What part of the body can be treated?

Fractional laser treatment can be used on any part of the body, but is particularly useful on the face, neck, chest and hands and eye area as well.

How long does the treatment take?

A treatment usually takes 20-60 minutes+ the time for anesthesia. A full face treatment usually complete in 15-30 minutes.

Does the fractional laser treatment hurt?

As the concentrated fractional laser light energy is absorbed by the cromophore turns into heat energy what consequently creates a sensation. Some areas may be more sensitive than others. We use topical anesthesia to lessen the sensation, what is described by most patients as not painful just mild and well tolerable discomfort what feels much like a rubber band snap or pin-prick with heat. On a 10 point pain scale usually it is described as a "pain" level 2.

What are the pre treatment instructions?

On your scheduled consultation a trained professional (qualified laser technician) will fully examine and evaluate your skin. Please arrive to your consultation not wearing makeup. Men are advised to shave if the face to be treated. Do not wear antiperspirant, perfume, cosmetics, body lotions, self tanning lotions on the area to be treated. Pre treatment you should not drink alcoholic beverages, because alcohol can draw water away from cells thus elongating healing process. Regeneration of cells can be enhanced by taking Vitamin C (500mg) daily, and homeopathy products may also help.

How is the fractional laser treatment carried out?

The treatment is carried out by our qualified laser technician. On your scheduled consultation a trained professional will evaluate your personal situation (if you are a candidate for this treatment) and advise you on the process. You will be informed in detail about the treatment, pre care and aftercare procedures.

The treatment begins by cleansing and disinfecting the treatment area. A topical anesthetic cream will be applied on your skin for 30-60 minutes prior the actual treatment. During the laser treatment you MUST wear protective eyewear! Our laser technician then sets treatment parameters on the laser machine and proceeds with an initial test shot to determine suitable energy settings. The scanner is placed in contact with skin surface and the laser light emitted onto the skin. The scanner is then moved to the neighboring area and the laser shots are repeated until full coverage of the treatment area is complete. Cooling, aftercare and sun protection takes place finishing the treatment.

What will my skin look like after the fractional laser treatment?

After the treatment your skin might look and feel more sunburn, also might feel warm and tingly while slight edema can occur. These usually resolve within 24-48 hours. The sunburned type burning feeling is usually gone in 90 minutes to 3 hours post treatment. About 3 days post treatment the skin will have a bronzes look and start to peel and slough off. As the skin’s natural healing takes time pinkish color will appear at the treated area for 5-7 days. This can be easily covered by makeup after the skin has totally healed and peeled. You might experience dryness on the treated area and for proper healing you must regularly moisturize with the recommended aftercare products to promote healing. You will experience immediate tightening of the skin post treatment, which will eventually turn into skin lifting effect. Following the aftercare instructions is STRICTLY your own responsibility and if you do not follow the aftercare instructions you do that at your own risk. When will I start to see results?

As we explained before, fractional laser treatment is such a special laser treatment that has both instant effects and positive long term results. The process of collagen remodeling induced by the laser is progressive, so even after 1-3-6 moths of treatment the skin will look better and better with each day.

Just after the first treatment the skin will look better no question. After the skin has healed and damaged cells are removed and new healthy cells are produced you will start to notice the textural changes, as your skin becomes velvety smooth and will be optically more even. After the peeling has completed the overall complexion shows a refreshed and more defined look, the pigmentation starts to fade and the wrinkles smooth out, contours are lifted. When treating atrophic scars and acne scars, results are visibly seen as the collagen remodeling happens. As depth of the scars become shallower with repeated treatments they eventually fade to an even and smooth young healthy looking skin. How many treatments are needed to see promising results?

Since every patient is different an initial consultation is necessary to determine treatment goals and to set up a treatment plan. We customize the treatment course to your personal individual needs. The number of treatments depends on the severity of the condition and the sensitivity of the skin. Usually a series of 3-5 treatments are necessary for optimal results. After just one treatment visible change can be seen after the skin heals. Outstanding, results (50-70%) can be achieved after just one treatment depending on the treatment settings. Lucky individuals can have their aesthetic skin problem solved after an initial treatment, however individual results may vary. Usually we recommend a minimum of 3 laser sessions. Depending on the severity of the condition the fractional laser treatment can be a stand alone treatment or combined with microdermabrasion, laser skin resurfacing treatment, photorejuvenation or chemical peels for best results. Our experienced staff will introduce you to suitable cosmetic products that further help by a boost of regenerating- nourishing and soothing active ingredients. For optimal outcome and minimizing possible risks and side effect you must strictly follow the given aftercare instructions!

How often can I be treated?

Treatments are usually performed 4-6 weeks apart for best results. However spacing between treatments depends on personal individual factors.

What can I do to maintain results?

Make sure to use suitable sunscreen when out in the sun! Sun protection is especially important after treating wrinkles, sun damaged and photo aging skin. It is advised to protect your “investment” (you took the time and money to have your skin become more beautiful) and use good quality skincare products and sunscreen cream (min SPF30 !). When out in the sun wear a hat, excessive sun exposure and excessive use of tanning beds can promote skin aging. Keep a good home skincare regimen and use products that are rich in active ingredients matching you skin type. If you are not sure, do not hesitate to contact our staff for more information.

Indications:  Fundamental base for the photoprotection of normal and dry skin. Exposure to UV radiation as the main factor responsible for skin aging and the occurrence of post-inflammatory hyperpigmentation, mesoestetic recommends its daily application, regardless of the treatment applied.

It has a chance to achieve best results that your skin will need pre treatment. For example a diamond microdermabrasion or an ultrasonic facial cleansing or PTF light based treatment to prepare your skin for the laser. What happens with my skin after the fractional laser treatment?

Post treatment 1-3 hours:

Right after the treatment, your skin looks and feels sunburned. It feels like you were out in the sun all day. May look red and have a warm feel also edema is possible. In general increased edema present under the cheeks, and eyes are normal phenomenon. To help lessen the edema, it is recommended that you sleep elevated the first day, use 2 extra pillows! It is very important to strictly follow the given after protocol, clean and moisturize the skin regularly with the aftercare products and use sunblock! You may feel discomfort. If so, in the first 48 hours cold compresses and ice packs as directed may be used. If needed you may use painkillers (usually patients don’t need it, it is listed here just in case) the first 48 you are only allowed to take for example: Rubophen, Panadol, Efferalgam. Ibuprofen such as Nurofen, Advil, Algoflex, Mortin can only be used 48 hours post treatment

 

1-2 days post treatment:

The skin will look red bronzes or dark, slightly raised, slight edema. You may experience warm feel to the treated area and tenderness. The texture may feel like sand-paper. Edema can intensify intermittently under the eye nose and cheek area. If edema is present, make sure to peel elevated as mentioned above. It is very important to strictly follow the given after protocol, clean and moisturize the skin regularly with the aftercare products and use sunblock!

 

3-7 days post treatment:

Usually the skin of the facial area heals quite quickly. Regularly the peeling starts 3 days post treatment. The skin will have a bronzes tone. The color change represents the microthermal zones coming to the skin surface. Under there are healthy new skin cells. Peeling and pinkish skin color is a good indication for healing. Peeling of the skin is continuous for 5-7 days. The skin on neck and décolleté, arm to peel completely will take longer. Most people will experience peeling to start around the nose and mouth. You are strictly NOT allowed to scratch, rub or pick the skin!  Make sure to wear a good coat of sunscreen and use the hydrating aftercare products regularly.

 

Upon completion of peeling:

Congratulation to your new refreshed skin! Your skin now feels silky smooth, it is less pigmented, the pore size lessened, the skin tone has visibly improved. If there are any leftover pigmentation slight wrinkles, or scars, please note that the positive effects are continuous and what you see is just the instant result, which will improve over time. After months your skin will benefit more due to collagen regeneration and the next treatment will enhance your overall complexion. Depending on personal individual factors there might be differences in healing between treatments.

 

What are the aftercare instructions?

5-7 days post treatment the treated skin will go through healing process. On your initial consultation a trained professional will introduce you to the laser treatment and answer all your questions about this treatment. You also will be informed about the aftercare instructions that will guide your healing and help to minimize the occurrence of side effects. Good to know that upon irritation of treated site (any harm to epidermis) germs bacteria can enter the skin, therefore avoid mechanical irritation! Avoid scratching rubbing and picking the skin, since it can cause permanent scarring. Also can higher the risk of complications. If the aftercare instructions are not followed carefully color changes in skin pigment may occur that can be short term effect but also can be permanent. Hypopigmentation, when the skin looses its color or hyperpigmentation may occur, when the skin becomes more pigmented. PIH effect may be seen in susceptible individuals.

Aftercare instructions:

During your initial consultation you will receive detailed information of aftercare instructions.

  • Make sure to protect your skin with a sunscreen with at least SPF 30 every morning before going outdoors for the next 4 weeks to avoid sun induced brown discoloration of the skin!  VERY IMPORTANT!
  • Do not suntan or use the tanning bed and do not use self tanning lotions for 4 week AFTER the treatment!
  • For couple of days after treatment, the treated area will be more sensitive, therefore do not take hot baths, massage and avoid the sauna! Avoid rubbing irritation and scratching the skin!
  • Until the redness on treated area faded you are not allowed to shave!
  • To achieve good results post-treatment, make sure to follow the aftercare instructions you were given.
  • For even better results consider using the recommended topicals and creams by our specialist.

What are the contraindications?

Certain skin conditions, health irregularities, and medications can impact whether it is safe for a person to receive laser treatment.

Try to avoid:

  • Sunburn, suntan, spray-tanning within 4 weeks.
  • Skin trauma, lesions, sores or open wounds, contagious skin diseases.
  • Active Herpes in the area to be treated.
  • Breastfeeding and/or pregnancy.
  • Fever, high temperature.
  • High blood pressure, heart conditions, epilepsy or other seizures chemotherapy, Untreated diabetes, Autoimmune conditions.
  • Photo-sensitive topical creams or medication and/or herbs, (Roaccutane, Accutane, Sotret within 6 months) Stop using Retin-A®, Renova® (or other retinoid containing products), exfoliating sponges/scrubs, glycolic, alpha and/or beta hydroxyl acid at least 2 weeks before your treatment. These products/medications increase photosensitivity and can increase the likelihood of complications.
  • History of keloid or hypertrophic scarring.
  • History of moles or skin cancer, melanoma.
  • Patient with unrealistic expectations.

If you are under 18, you can only receive treatments at the Szexepil Szalon if you present written consent from your parent or guardian.

 

I’m interested in wrinkle treatment and skin tightening especially my eye area. Gravity took toll on my eyelids and they make me look tired all the time. I wish to have my eyelids lifted and crows feet lessened, and less wrinkles on my face. I definitely do not want surgery yet. Is this fractional laser treatment good for me?

Yes! After a few treatments you can achieve visible long lasting results. Some cases laser treatment may be a good alternative to blepharoplasty. Repeated laser sessions brighten up you eyes, lids are lifted and youthful look is restored. During eye area treatment the usage of protective laser eyewear is essential. I noticed vertical lines at my upper lip. As a former smoker it may developed because of me smoking. I feel too young to have these lip lines. I want to erase them. Is this treatment right for me?

Patients need to understand that this treatment have a progressive effect. This means that after each treatment your skin will produce more and more collagen filling up the “holes” what takes time. Concerning having this treatment you should have realistic expectations. Be a patient patient, and expect your skin to look more healthy after each laser session. After the peeling and healing has completed you will experience skin textural changes and enhancement with each day. Due to the progressive effects what you will see is your skin look better after 2 months than 2 weeks post treatment. Minimum 3 treatments are required to boost up collagen synthesis that much to promote the skin’s natural regeneration process to have the scars cleared up. Bothersome scars can be faded and removed but this takes time. Collagen regeneration takes time.Get minimum 3 laser treatments and be patient and wait for your skin to naturally regenerate itself repairing the skin structure. I had an accident some years ago and I left with scars. I had a pressure ulcer and the pressure sore left me with a scar. First of all it is at a very bothering site, and when I lean down it tenses and pulls the rest of my skin, and I don’t like the discoloration. It has a darker color and the texture is much harder and denser than my normal skin. I would be so happy even for the slightest improvement. Can we do something about it with this laser treatment?

I had plastic surgery 10 years ago. I had blepharoplasty and the scar on the lower lid not only have not faded but the scar itself is prominent.
